Eileen F. Cusack, LCSW, ACSW, CPH,

Director, Clinical Services at Physicians' Practice Enhancement (PPE). Eileen has worked in the health care industry for over 30 years with clinical and administrative oversight of various programs and services across the continuum of care. Her specializations include case management, discharge planning, quality and systems improvement, behavioral health and end of life/palliative care. Eileen has held the posts of Director of Care Coordination, Administrative Director of Systems Improvement and Director of Case Management and Social Work at teaching and community Hospitals in the New York/New Jersey/Pennsylvania area.

Eileen holds a Masters in Social Work Administration from Rutgers University and a Bachelors in Social Welfare from Temple University. She completed a training program in personnel management from the Irish Ministry of Labor. She is a NJ Licensed Clinical Social Worker, a member of the Academy of Certified Social Workers and is a Certified Professional in Healthcare Quality. Eileen is also a Certified Social Work Supervisor.
